Adult and Family Services

Adult Protection:
APS investigates allegations of physical and sexual abuse, caretaker neglect, exploitation, harmful acts (collectively referred to as "mistreatment"), and self-neglect of at-risk adults. APS offers protective services to improve the health, safety, and welfare of at-risk adults experiencing mistreatment or self-neglect. APS uses community-based services and resources, health care services, family and friends when appropriate, and other support systems while protecting the at-risk adult's right to confidentiality, self-determination, and least-restrictive intervention. APS Mandatory Reporting:
Certain professionals are required by law to report physical abuse, sexual abuse, caretaker neglect, and exploitation (termed “mistreatment”) that is observed or that the reporter becomes aware of when that mistreatment is occurring to an at-risk elder or an at-risk adult with an intellectual or developmental disability (IDD). These mandatory reports must be made to the law enforcement agency where the mistreatment occurred within 24 hours of witnessing or becoming aware of the mistreatment. Vintage:
Vintage is the Alpine Area Agency on Aging. They provide services for aging well in Colorado. Vintage is the Alpine region’s aging expert – ensuring that all of us have access to the support, services, and resources we need when we’re older.

Services include vouchers for transportation assistance, dental/vision assistance, options counseling, nutrition programs, caregiver support, respite services, evidence-based health programs, volunteerism, and educational programs. They serve Eagle, Grand, Jackson, Pitkin, and Summit counties in Northwest Colorado. See the website for more information.

Care Navigation:
The Care Navigation Program offers lower-level direct case management for adults ages 60+ who may need coaching or some assistance navigating resources and/or obtaining support services. Care Navigation services are completely voluntary and confidential. Our goal is to help create and maintain a safe and healthy environment that supports the highest possible level of independence for the senior adult and to enrich the lives of those experiencing life changes. This program is managed by Vintage with Pitkin County providing the direct case management outreach. Here is the link to make a referral for Care Navigation Case Management.
