Request for Reactivation of Expired OWTS Use Permit Application

  1. This form should be used for requesting reactivation of an expired ISDS or OWTS Construction Permit. If there have been any significant changes or deviations from the information submitted and accepted by this Department with regard to the expired permit, a new application and fee will be required. Permits issued under prior regulations may only be reactivated under limited circumstances.
